Health Service Forms

All incoming students must complete and return the Required Medical Forms prior to their arrival at OWU.

All athletes will be required to have a physical with one of the OWU physicians prior to their participation in sports. The following forms are needed for their physical and can be brought with them for their exam.

• Athletic Form – First-Year (PDF*)
• Athletic Form – Returning (PDF*)
• Student Athlete Travel Insurance Form (PDF*)
